What Medical Evidence Do You Need?

Most veterans fall into one of three situations. Find yours below.

01  ·  Increase
Disability Benefits Questionnaire (DBQ)

If an existing service-connected condition has gotten worse, a DBQ documents its current severity, symptoms, and functional impact -- the kind of detail the VA looks for when evaluating an increase.

Get a DBQ
02  ·  Secondary Condition
Nexus Letter + DBQ

If one condition may be connected to, or made worse by, an existing service-connected disability, a Nexus Letter addresses that medical relationship. A corresponding DBQ is included when clinically appropriate.

03  ·  Denied Claim
Medical Evidence Review + Attorney Referral

If your claim was denied, you may need more than new evidence. We'll review what you have and, when appropriate, connect you with an accredited VA attorney for Supplemental Claims, Higher-Level Reviews, or Board Appeals.

Medical Evidence

Physician-Prepared DBQs

A Disability Benefits Questionnaire documents your diagnosis, symptoms, severity, and functional impairment in the format the VA uses to evaluate claims. It's most commonly relevant when you're seeking an increased evaluation of an existing service-connected disability.

  • Mental Health
  • Migraines
  • Back & Neck Conditions
  • Knee & Shoulder Conditions
  • Sleep Apnea, GERD, IBS
  • Radiculopathy & Other Conditions

Independent Nexus Letters

A Nexus Letter is an independent medical opinion addressing the relationship between a claimed condition and your service, an existing service-connected disability, or another qualifying event. The medical provider reaches this opinion using independent clinical judgment.

  • Secondary Service Connection
  • Direct Service Connection
  • Aggravation Claims
  • Combined with a Corresponding DBQ

Questions

Frequently Asked Questions

What is a DBQ? +
A Disability Benefits Questionnaire is a standardized medical form the VA uses to document a condition's diagnosis, symptoms, severity, and functional impact. It's commonly used when seeking an increased evaluation of an existing service-connected disability.
What is a Nexus Letter? +
A Nexus Letter is an independent medical opinion addressing the relationship between a claimed condition and another condition, event, injury, exposure, or circumstance. The medical provider reaches their opinion using independent clinical judgment.
What's the difference between a DBQ and a Nexus Letter - do I need both? +
A DBQ documents severity and symptoms for a condition. A Nexus Letter addresses the medical relationship between conditions. Many secondary-condition claims need both. We'll help you figure out what applies to your situation during intake.
How much does a DBQ or Nexus Letter cost? +
One document is $750. Two or more documents are $500 each. For example, a DBQ plus a Nexus Letter together is $1,000 total.
Why is the price lower when I need multiple documents? +
Preparing multiple documents together is more efficient for our physician network, and we pass that efficiency on to you.
Do you take any of my VA backpay? +
No. We charge flat, upfront pricing for medical evidence. We never take a percentage of your compensation or backpay.
Do you need access to my VA.gov account? +
No. Standard medical evidence services do not require your VA.gov or ID.me login. You keep your credentials to yourself.
Can you guarantee my VA claim will be approved? +
No. Medical professionals exercise independent medical judgment, and the Department of Veterans Affairs ultimately determines service connection, disability evaluations, effective dates, and entitlement to benefits.
Can Seven Principles submit my medical evidence to the VA? +
You can submit your completed evidence yourself, or use our courtesy VA evidence fax submission service where applicable.
What happens if my claim was denied? +
We'll review your situation and, when appropriate, connect you with an accredited VA attorney partner for a Supplemental Claim, Higher-Level Review, or Board Appeal.
Do you handle Higher-Level Reviews or Board Appeals directly? +
Legal representation for appeals is handled by our accredited VA attorney partners, not by Seven Principles directly. We focus on medical evidence and make the introduction when it's appropriate.
What is a DIC claim, and who can file one? +
Dependency and Indemnity Compensation (DIC) is a VA benefit available to certain surviving spouses (and some dependents) of veterans. Eligibility depends on the circumstances of the veteran's service and, in some cases, cause of death. Our filing fee for helping a surviving spouse prepare and file a DIC claim is $1,500.
What is a discharge upgrade, and how much does it cost? +
If you separated from service with a discharge status other than Honorable, you may be able to petition the appropriate military discharge review board for an upgrade. Our filing fee for preparing and filing a discharge upgrade petition is $1,500.
